This aimed to determine the causes of mathematics anxiety of the students and their relationship to some selected variables. A descriptive correlational method employs using a validated instrument and percentage, mean, weighted mean and spearman Rank Order Correlation as statistical tools. It was revealed that the extent of students' perception of the following factors that may cause the math anxiety was high: teachers' instruction, curriculum standard, and subject difficulty. It was also revealed that their mathematics anxiety was high, and their performance was in satisfactory level. There was also a significant relationship between the prior performance of the students in mathematics and the teachers' instructions and curriculum standard.
